Countries that block communications usually have several things in common:
They are not democratic
They have poor human rights records
They do not believe in freedom of speech
They do not have a court system as we in the USA or Europe define it
They have really nasty jails
They often kill people who say things they don’t like
All of which means that bypassing a countries blocks on the internet while using VoIP is almost a sure way to get someone in trouble. There are no truly secure voice scrambler systems available on the open market. If they claim to be secure, you’d better check. It’s most likely a scam
If if the voice signal is truly scrambled, it is still going to be very hard to hide the IP address in the blocked country. Assuming the person on that end is skilled and uses a system such as TOR to hide behind, he may be able to hide for a time. But the amount of data and the data rate needed for decent VoIP usually prohibit using such relay systems.
Your best bet for secure coms in a blocked country is a satellite phone on a directional antenna with an external scrambler. The Internet is a party line. If you must use it, use short e-mails that are encrypted twice with different keys or systems and never use the same key twice.
